Responsibilities

• Good communication and listening skills to identify social, emotional, and financial needs of short‑term skilled patients and long‑term care residents.
• Oversee daily functioning of the Social Service Designee.
• Provide services to residents/families to assist with adjustment and problem‑solving regarding social, emotional, and financial needs.
• Assist in implementation of room and roommate changes.
• Attend Medicare meetings or designate social service designee.
• Coordinate discharge planning for patients, arranging follow‑up community services, home health referrals, and alternative placements.
• Advocate for residents and families regarding resident rights issues.
• Conduct in‑services to educate staff on emotional and social needs of residents and methods for dealing with residents and families.
• Attend administrative staff meetings, morning report, performance improvement meetings, resident care‑related meetings, and quarterly staff meetings.
• Maintain departmental records/documentation as required.
• Coordinate/update resident care conference schedule or designate social service designee.
• Work with front office to assist residents/families with Medicaid approval/reapplication process.
• Provide Medicaid pending/patient liability related follow‑up as directed by Facility Administrator.

Qualifications

• Meets requirements for Social Services Director for a skilled nursing facility or holds an active social worker license in the state of the facility.
• Long‑term care experience preferred or willingness to learn.
• Resident advocate within the structure of communal residential living.
• BS/LSW/MSW required.
